Following of the release of De Dana Dan in December 2009, Priyadarshan announced that he was working on the script of a film on AIDS with Aamir Khan in the lead role .[1] The pair regularly discussed the script and Khan had agreed to fund the project, with Priyadarshan revealing he was still scripting the film as of February 2011.[2] However, the director took longer than expected to complete work on the script and by May 2012, Khan and Priyadarshan chose to shelve the project.[3] The director considered bringing in Akshay Kumar to reprise the lead role instead, but the project failed to materialise.[4]

In July 2015, Priyadarshan announced that he would instead make the film in Tamil with Prakash Raj and Sriya Reddy in the lead roles. Santosh Sivan was selected to be the film's cinematographer, while Beena Paul and Sabu Cyril were picked as editor and art director respectively. He also revealed that the film would be produced by his former associate A. L. Vijay's Think Big Studios and that the film would be made at a cost of 2 crore rupees, with the cast and crew getting a salary but not at their market rate.[5] Prabhu Deva later announced that he would co-produce the film alongside Amala Paul, who was assigned as the film's main production representative by Think Big Studios.[6] Vijay also suggested that Nassar and Ashok Selvan could be cast in the film, and Priyadarshan took the actors on board the project. Sameer Thahr later replaced Sivan, a few days before the start of the shoot. The team began filming without an assigned music composer and had initially hoped to convince A. R. Rahman to work on the film after a screening, but the composer was busy with other assignments.[7] His unavailability meant that the team signed on Ilaiyaraaja to work on the film's background score.[8]

The team shot portions in August 2015, with Prakash Raj revealing that the film had been titled Sila Nerangalil.[9] The film was shot within twenty five days with Sync Sound by Prince Anselm. After production was completed, Amala Paul announced that the film had been retitled as Sila Samayangalil.[10]
